    • At Fisher-Gentry Eye Care, we accept both vision plans and medical insurance. We also accept payment from flexible spending accounts (FSA) health savings accounts (HSA), Care Credit, cash, checks and most major credit cards.

    • Medical/Health Insurance plans protect you against financial losses due to unexpected eye injuries or disease. In some cases your exam will be covered by your medical insurance and/or may be coordinated with your vision plan to maximize your coverage and minimize your out of pocket expense.

      • Note: It is possible that a routine eye exam could become a medical exam based on the findings of the doctor.

    • Insurance can sometimes be confusing. Don’t worry – we’ve got you. Because we provide both medical and routine eye care, we accept a number of insurance plans to help cover the cost depending on your specific needs. If you have vision insurance and medical insurance, we recommend that you bring information for either or both. Our team will handle the rest.

    • We accept cash, major credit cards (Mastercard, Visa, American Express, and Discover), and CareCredit for any necessary payments.

    • Vision plans typically cover routine eye exams and provide a specified dollar amount or discount toward the purchase of eyeglasses and/or contact lenses. Most plans require a co-payment for an office visit and may involve additional costs that are the responsibility of the insured patient. Our office will work directly with your vision insurer to submit claims securely and electronically. Some policies offer discounts on refractive surgery. We accept the following vision plans:

      • EyeMed

      • Spectera

      • Davis Vision

      • VSP

      • Humana VCP

  • Frame Warranties*: Frames are under warranty for one year from the date of purchase. The first replacement will be free of charge. The second replacement will be a $25 processing and shipping fee. Any additional replacements beyond that will be at the regular priced frame.

    *Does not cover lost or stolen

     

    Lens Warranties: 

    Most of our lenses come with a one year, scratch resistance (not scratch proof) warranty.  Scratched lenses, under warranty, may be replaced only one time with a processing and shipping fee of $25.


    Under certain circumstances the lens warranty is UNAVAILABLE:

    These situations may be, but not limited to: Maui Jim Sunglasses, mirror coatings, safety frame orders, CR-39 plastic lenses without anti-reflective coatings. In some instances, insurance companies may ask that you purchase the scratch warranty at the time of ordering glasses in order to be eligible to use a one time lens replacement. Please see an optician with any questions or concerns you may have.

  • Thank you for trusting your vision care with Fisher Gentry Eye Care. When you schedule an appointment with our office, we set aside enough time to provide you with the highest quality of care. Should you need to cancel or reschedule an appointment, please contact our office as soon as possible as this gives us adequate time to schedule other patients who may be waiting for an appointment.  

    As of January 1st 2022, we will be implementing the following no show policy:

    • Any established patient who fails to show for an appointment will be charged a $25 fee.  This fee must be paid in full if the patient wishes to reschedule.  A second no show will result in a $50 fee.  If the patient does not show for a third time, they will be dismissed from our practice. 

    • Any new patient who fails to show for an appointment will be charged a $50 no show fee.  This fee must be paid prior to rescheduling any future appointments.  If a new patient does not show for their second scheduled appointment, they will be dismissed from our practice and no future appointments will be scheduled.  

    We understand that emergencies and sickness happen unexpectedly.  We ask that you make a reasonable effort to contact our office if you are unable to make your scheduled appointment.  Our office can be reached at (815) 468-2015 and there is a voicemail set up for after business hours. Messages left will be considered acceptable notice.
